The Hydrogen and Methane Breath Test is a precise and comfortable diagnostic tool.

It involves drinking a sugar solution (like lactulose) while fasting, followed by breath measurements of the gases produced by your intestinal microbiota.

Normally, hydrogen is exclusively produced in the colon. However, in SIBO cases, excess bacteria in the small intestine ferment the solution early, leading to elevated hydrogen or methane levels in your exhaled breath.

The normal transit time is approximately 90 minutes, and tracking these levels helps your doctor understand your gut environment and determine the best treatment course.

The accuracy of this test depends entirely on your strict adherence to this timeline to preclude falsely elevated baseline gas levels:

  • 4 Weeks Prior: Strictly no antibiotics, stool softeners, or hardeners. Colonoscopies or fluoroscopy procedures are also contraindicated.

  • 2 Weeks Prior: Stop all probiotic and prebiotic supplements.

  • 3 Days Prior: Avoid fiber supplements, Imodium, Pepto-Bismol, antacids (PPIs), and any medications containing fructose flavors or lactose fillers.

  • 24 Hours Prior (Prep Diet): You may ONLY eat: baked/grilled chicken, turkey, fish, plain white rice, eggs, and water. STRICTLY AVOID: Milk/dairy, fruits, vegetables, grains, nuts, legumes, honey, and sugar-free foods. (If you suffer from constipation, follow this diet for 2 to 3 days).

  • 12 Hours Prior: Complete fast (only plain water is allowed).

  • Day of the Test: No smoking, exercising, chewing gum, or using denture adhesives. You may brush your teeth two hours prior

The lab establishes a baseline reading and collects subsequent breath samples at specific intervals. A positive test suggesting SIBO is indicated by a rise of $ge 20$ ppm from the baseline in hydrogen by 90 minutes, or a peak methane level of $ge 10$ ppm at any point. Results must be reviewed with your physician alongside your clinical symptoms.

Normal results do not rule out all gastrointestinal conditions, and abnormal results require medical interpretation.

  • Diabetic Patients: Overnight fasting may not always apply, and blood sugar must be controlled. Lactulose is preferred, and the test may take 3-4 hours due to potentially delayed gastric emptying.

Is fasting required?

Yes, a 12-hour fast is required before the test; only plain water is allowed.

How does delayed gastric emptying affect the test?
